How can I become an
ER holder ?
Fill in a proposal form. Attach the following
and send to APA office together with payments:
(a) Two passport
photographs
(b) Copy of the member’s national
Identification Card or Passport (for adults)
Once the application is accepted the insured
receives an acceptance letter and his member Photo-ID card.
