Reimbursement Claims

Complete and sign the APA Health claim form, the attending physician must also complete and sign the claim form
All original receipts should be attached to the claim form and All copies of the doctor’s prescription and laboratory request forms must be attached to support pharmacy and laboratory cash receipts
For Inpatient, All original receipts and invoices to be attached to the claim form. A discharge summary must also be attached for inpatient claims. Full medical reports must be submitted to APA Health, where required.
A fully documented claim will be settled within 10 working days
