Out-Patient Claims

  • Present  your membership photo card at the service provider’s desk for identification
  • The service provider will check and confirm membership and treatment will be rendered
  • Complete and sign the APA Health claim form (available at the service provider’s facility) the attending physician must also complete and sign the claim form.
  • Please ensure that you sign the claim form after the physician/consulting doctor has completed it.
  • Be sure to complete a separate form for each treatment and for each person, and All bills will then be sent directly by the service provider to APA Health for settlement
  • APA recommends that you acknowledge the bills by asking to sign for them. We recommend that you scrutinize all bills and certify their correctness before signing
